Application Procedure
Register your e-mail address
  • Register your e-mail address and then you will receive URL for application to the registered address.
  • All of information shall be informed to the registered address.
Access to the URL
  • Access to the URL for application and confirm the required information.
  • This URL is effective until the completion of your registration.
Fill in applicant information and download the Application form and Letter of recommendation
  • Fill out the Application-form and the Recommendation letter with signature, and upload those documents after convert into the PDF format.
Finalize your registration
  • After completion of filling in the information and uploading the application documents, you click the button of application.
  • If everything is OK, you will receive the registration number to your registered e-mail address.
  • Unless you receive the registration number within one day after your application, you have to inquire to Kobayashi Foundation for Cancer Research or you fail the registration.
